session-flow 2.0.3: only open work in the sequence
Deferred and cancelled work leaves SEQUENCE.md too. The generated backlog contains only open work, and retired identities stay taken.
Changes
+Only captured, accepted, active and blocked items appear in the generated sequence. Deferred and cancelled items leave it along with their comment lines.
+Hidden items keep their records and task views. Their identities are never reused, and reopening deferred work restores it in its stored display order.
+Legacy import still accepts deferred entries and verifies them, including records that stay out of the generated view.
