Evidence-based verification
A pass that cites a file and line, a command's output or a test name for every claim, and treats missing evidence as failure.
For each claim in the specification, the pass designs a test meant to disprove it; the claim survives only if the attempt to falsify it fails. Running the code counts, and reading it does not.
The rule that does the work is the one about absence: no evidence means not verified, rather than probably fine.
