One dated entry per ended agent session: what was asked, what was done, the evidence for it, and what is still open.
Entries land on whichever backend a repo is mapped to, a Notion project page or a dedicated GitHub issue reused for the repo's whole history, written by a hook when the session ends rather than by hand.
The backend is the review surface. The repository stays the source of truth.
